The usual finding is heavily concentrated: a small number of causes produce most defects, which makes the response manageable.
Grouping needs a stable category set. Free-text causes cannot be aggregated and produce a record that is read once.

Each attribute is one of the framework’s shared types. What a date is, and how it is written, is defined once for the whole model — the note here says what it means in this particular place.
